Patient privacy matters. This page explains how we protect your health information, how you may
request records, how to contact our licensing authority, and how to file a privacy or consumer
complaint.

Notice of Privacy Practices
We may use and disclose protected health information for treatment, payment, and health care
operations. We may also use or disclose information when required or permitted by law. Other uses may
require your written authorization.

How to Request Your Records
Submit a written records request or authorization by mail, fax, or in person. Please include your full name, date of birth, phone number, records requested, date range, delivery method, and the recipient’s name and destination if records are being sent to someone else.

Fees
For patient access requests, any fee charged will be reasonable and cost-based as required by HIPAA. We will notify you in advance if a permitted fee applies. Additional fees may apply for certain third-party, legal, insurance, imaging, mailing, affidavit, or special requests when permitted by law.

File a Consumer or Privacy Complaint
You may file a complaint with our Privacy Officer, the Texas Board of Chiropractic Examiners, the Texas Attorney General Consumer Protection Division, or the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
